Office Visits
We ask that you please make an appointment for every office visit. We strive to keep every patient's wait time under 15 minutes and we do occasionally accept walk-in patients if our schedule can accommodate. If you have an urgent need, it is likely we can accommodate you with an appointment the same day.
Please bring your insurance card and co-pay to every office visit. To help us best serve all patients, please be on time for your scheduled appointment.
New Patients
If you are a new patient, please arrive at least 15-20 minutes before your scheduled appointment so we may verify your insurance and ask you to complete your new patient forms (you may also download the forms below and bring them to your first appointment).
When coming to your first appointment, please bring your medication list, past medical history, insurance card, prior medical records (if available), your immunization record, and your insurance co-payment.
*Parents, please bring your children’s immunization records.

Insurance Partners
We participate with many of the major health insurance carriers, such as Medicare, BCBS, SmartHealth, Priority Health and most commercial carriers, to name a few.
Prior to your appointment in our office, it’s a good idea to check if care by our physician is covered by your health plan and to learn about your plan’s covered services, payment terms and referral policy. Co-payments and deductibles are expected to be paid at the time of your appointment. For your convenience, our office accepts MasterCard, Visa, Discover, personal checks and cash as payment for services.
Regardless of your insurance coverage, you are responsible for payment for the services performed. Our staff will be happy to submit initial claim forms to your insurance carrier. If you are unable to show proof of coverage, we expect payment for services at the time of your office visit.
